Health and Wellbeing
Navigator

Six short questionnaires help explore how physical wellbeing, stress, sleep, relationships, lifestyle and personal direction may be connected.

Profiles, email addresses and answers remain in your browser. Nothing is sent without your action.
Important. This is a self-assessment tool, not a diagnosis. It does not recommend medicines, supplements, doses, or changes to prescribed treatment. Seek urgent help for sudden deterioration, severe pain, breathing difficulty, altered consciousness or immediate danger.

Symptoms that require medical attention

If you have any of the symptoms listed below, do not proceed to the questionnaires: first contact the indicated clinician or seek urgent medical care. A questionnaire cannot establish the cause of such a condition and must not delay an in-person assessment. If none of these symptoms is present, you may proceed to the questionnaires.

Heart and circulation

Chest pain or pressure, marked breathlessness, fainting, or sudden palpitations with feeling unwell.

Emergency care; primary-care clinician or cardiologist.

Nervous system

Sudden one-sided weakness or numbness, speech difficulty, facial droop, or a new extremely severe headache.

Emergency care; neurologist.

Surgical conditions

Severe or worsening abdominal pain, a rigid abdomen, repeated vomiting, or blood in vomit.

Emergency care; surgeon.

Cancer warning signs

Unexplained weight loss, blood in urine, stool or sputum, a growing lump, or a wound or ulcer that does not heal.

Primary-care clinician; then an appropriate specialist or oncologist.

Endocrine system

Persistent thirst with frequent urination, rapid weight change, or marked unexplained weakness.

Primary-care clinician or endocrinologist.

Genitourinary system

Urinary retention, blood in urine, severe flank or groin pain; unusual bleeding or severe pelvic pain.

Urologist or gynaecologist; emergency care for severe symptoms.

Children

Breathing difficulty, seizures, unusual drowsiness, dehydration, or rapid deterioration in a child.

Paediatrician; emergency care for severe symptoms.

Mental health

Thoughts of self-harm, loss of contact with reality, or an immediate threat of violence.

Emergency care or a mental-health crisis service.
